First Visit
Welcome to the Indian Family Health Clinic.
Please understand we are not a walk-in clinic, emergency room, or comprehensive hospital. You must either call and make an appointment or come in and schedule an appointment.
We want to make your first visit with us and the paperwork involved go as smoothly as possible. On your first visit to the clinic you have to register with us, so, to prepare you for your first time in, please bring the following with you:

Social Security Card

Photo ID

Proof of Enrollment

Any health insurance coverage including Medicaid/Medicare

Front Office
When you come in to the clinic, it is necessary for us to check your chart to make sure all information is updated and the required paperwork is in your chart. Please understand that some paperwork is required to ensure that we receive federal funds from the Indian Health Service.
As a reminder, any patients under 17 will not be seen unless accompanied by an adult for each and every visit unless otherwise specified. We do have consent forms, but the parent will need to come in and fill it out along with any necessary paperwork. We will not give the consent form out to underage children to bring home and have parents fill out.
Medication Pick-up Times
Monday, Tuesday, Thursday - 3:00 p.m. - 5:00 p.m.
Friday - 3:00 p.m. - 4:00 p.m.
If you drop off your prescriptions by 5:00 p.m. on Tuesdays, they will be transported to Browning to be filled on Thursday of that week. Any prescriptions or refills dropped off after that will not be sent until the following week.
Transportation for Patients
We will provide transportation to Browning for patients who need healthcare services that are beyond the scope of a primary care clinic. This transportation will coincide with our prescription/medication pick-up. Every Thursday our van leaves from the clinic at 5:30 a.m. Patients who wish to utilize this service must first schedule with the front office so we can accommodate everyone's needs. The van schedule is usually full 2 weeks in advance. Call for details. |
